Please note that this form enrolls you as a patient at Hauora Health.  At this point We can request your (and any children under 16 years) medical records from your previous GP Practice. This process can take 10 working days to complete, so please be aware that We cannot organise any prescriptions until we receive your notes. We will email or text you once they have arrived. For guidance it always is best to book your new patient appointment at this point as we will have all the relevant information
